儿童纵隔囊性占位性病变的临床特征及手术治疗单中心经验
Clinical characteristics and surgical strategies of cystic mediastinal lesions in children:a single-center retrospective experience

Abstract
Objective To explore the clinical manifestations and surgical strategies of mediastinal cystic lesions in children.Methods A retrospective analysis was conducted for the relevant clinical data of 52 chil-dren of mediastinal cystic lesions operated from January 2019 to October 2024.This cohort included 35 boys and 17 girls with a mean age of 5.17 years(10 days-14.17 years).The outcome measures included age,operative duration,intraoperative volume of blood loss,postoperative volume of drainage,length of hospitalization stay,ab-sence or presence of complications and intraoperative conversion into thoracotomy.Results The procedures in-cluded thoracoscopy(n=48),conversion into open thoracotomy(n=3)and median sternotomy(n=1).All cases were confirmed by postoperative pathological examination.There were 26 cases of foregut cysts(operative duration 99.35±66.15 min,intraoperative volume of blood loss 4.40±10.00 mL,postoperative volume of drainage 62.80±69.90 mL and hospitalization stay of 8.50±4.70 day),14 cases of teratoma(operative dura-tion 207.57±136.13 min,intraoperative volume of blood loss 96.00±217.00 mL,postoperative volume of drainage 215.40±215.50 mL and hospitalization stay 11.57±8.27 day),10 cases of lymphangioma(opera-tive duration 190.70±97.70 min,intraoperative volume of blood loss 18.00±33.00 mL,postoperative volume of drainage 366.90±807.10 mL and hospitalization stay 13.60±15.48 day)and 2 cases of thymic cyst(opera-tive duration 190.00±183.85 min,intraoperative volume of blood loss 50.50±70.00 mL,postoperative volume of drainage 232.50±314.70 mL and hospitalization stay 8.00±2.83 day).Perioperative complications occurred(n=5),including intraoperative hemorrhage(n=3),diaphragmatic eventration(n=1)and recurrent lym-phangioma(n=1).It stabilized after interventional treatment.There was no mortality.Conclusions Pediatric mediastinal cystic lesions may have diverse pathological types.Early diagnosis and evaluation of appropriate treatments based upon imaging characteristics are critical.Thoracoscopic resection is a primary treatment for mediastinal cystic lesions in children.关键词
