西南低卫生资源地区女性自然人群中高危型人乳头瘤病毒感染现状分析——基于四川省盐亭县的研究
Analysis on status of high-risk human papillomavirus infection among natural female population in low-resource areas in southwest China:based on investigation of Yanting county,Sichuan province

Abstract
Objective To analyze the infection status and distribution of high-risk human papillomavirus(hrHPV)in natural popu-lation from a low-resource area,Yanting county,Sichuan province,in southwest China.Methods From November 2017 to February 2019,local women aged 21 to 64 years were recruited in a cervical cancer screening program in Yanting County,Mianyang city,Sichuan province.After obtaining informed consent,cervical exfoliated cell specimens were collected for hrHPV nucleic acid testing and cyto-logical diagnosis.Colposcopy was performed in women with atypical squamous cells of undetermined significance(ASC-US)or worse(≥ASC-US),or HPV16/18 positivity,and biopsy was taken when necessary for final diagnosis.Univariate logistic regression was used to assess the pathogenic risk of different hrHPV genotypes.χ2 test was employed to compare the distribution of hrHPV infections of in-dividuals with different cytological and histopathological results.Cochran-Armitage trend test was used to evaluate the relationship be-tween hrHPV infection rates and the severity of cytological or histopathological results.Results A total of 6 175 women with a median age of 48 years were included.Among them,1 063 were hrHPV-positive,with an infection rate of 17.21%.The prevalence of cervical intraepithelial neoplasia grade 2 or worse(CIN2+)and CIN grade 3 or worse(CIN3+)was 1.12%and 0.55%,respectively.In general,the rate of single HPV16 or HPV18 infection was higher than that of HPV16/18 co-infection with other hrHPV.The rates of HPV16(χ2=14.15,P=0.007)and other hrHPV(χ2=40.62,P<0.01)infections were both statistically different among the age groups of 21-30,31-40,41-50,51-60,and 61-64 years,and the positive rates increased with age(χ2=5.60,P=0.006;χ2=33.39,P<0.01).Among the dif-ferent cytological results,the infection rates of HPV16(χ2=233.21,P<0.01),HPV18(χ2=33.48,P<0.01),and other hrHPV(χ2=291.01,P<0.01)were significantly different,and increased with the severity of the cytological diagnosis(all P<0.05).Among individuals with different histological diagnoses,the positive rate of HPV16 was higher in those with CIN2+and CIN3+,and the infection rates of HPV16(χ2=43.22,P<0.01),other hrHPV(χ2=43.20,P<0.01),and any hrHPV(χ2=99.97,P<0.01)were statistically different,and the infection rates of HPV16 and any hrHPV both increased with the severity of histological diagnosis(both P<0.01).Conclusions The prevalence of CIN2+and infection rate of hrHPV are relatively high in the natural female population in this area with low healthcare resource.HPV16 and HPV18 are predominantly detected as single infections.The hrHPV infection rate is associated with age,cytological diagnosis,and histological diagnosis.关键词
