早期营养干预对鼻咽癌放疗摆位精度的影响:一项历史对照研究
Effect of early nutritional intervention on accuracy of radiotherapy setup for nasopharyngeal carcinoma:a historical controlled study

Abstract
Objective To investigate the impact of early nutritional status changes and nutritional intervention on the precision radio-therapy of nasopharyngeal carcinoma(NPC)using cone beam CT(CBCT).Methods A total of 131 NPC patients undergoing radiotherapy were enrolled from the First Affiliated Hospital of Air Force Medical University.Among them,66 patients treated between January 2018 and July 2020 received no nutritional intervention(control group),while 65 patients treated between August 2020 and May 2023 received nutritional intervention(intervention group).Nutritional status and risk were assessed weekly using the patient-generated subjective global assessment(PG-SGA).Nutritional intervention was initiated in the intervention group when undernutrition(PG-SGA score≥2)or body mass index(BMI)<18.5 kg/m2 was detected,whereas the control group received standard clinical care without dietitian involvement.Treat-ment position verification was performed weekly using CBCT.Radiotherapy interruption rates and setup errors were compared between the two groups.Results The radiotherapy interruption rate was significantly lower in the intervention group than in the control group[6.2%(4/65)vs 28.8%(19/66),P<0.01].For the first four weeks of the radiotherapy,the setup errors in the x-axis direction showed no significant difference between the intervention group and the control group[(1.12±0.60)mm vs(1.20±0.64)mm,P=0.482],whereas significant dif-ferences were observed in the y-axis[(1.58±0.70)mm vs(1.85±0.62)mm,P=0.026]and z-axis[(1.61±0.65)mm vs(1.89±0.69)mm,P=0.020]directions.After plan revision,no significant difference in setup errors was found in any direction(P>0.05).Conclusions Early nutritional intervention improves treatment compliance,reduces radiotherapy risk,enhances setup accuracy,and ensures the successful delivery of precision radiotherapy in NPC patients.关键词
