Abstract
Objective To observe the preventive effect of intravenous pre-infusion of Nalmefene on the adverse reactions of epidural labor analgesia.Methods A total of 210 cases of primigravid women who received epidural labor analgesia from July to October 2023 in Yantai Yuhuangding Hospital,Qingdao University were selected as the study subjects.They were divided into control group(group S),observation group 1(group D1)and observation group 2(group D2)using the random number table method,70 cases in each group,and were given 5 ml saline,0.1 μg/kg Nalmefene and 0.2 μg/kg Nalmefene intravenously 10 minutes before the implementation of epidural anesthesia,respectively,and all of them were anesthetized with continuous epidural anesthesia of Ropivacaine(0.08%)+Sufentanil(0.5 μg/ml).The main observational indexes were the severity and incidence of nausea,vomiting,and pruritus within 24 hours after anesthesia,and the secondary observational indexes included the maternal pain scores within 24 hours after anesthesia,the newborn's Apgar scores at 1,5,and 10 minutes after birth,and the rate of admission to the intensive care unit(ICU)at 7 days after birth in the three groups.Results The severity of maternal nausea and vomiting and pruritus in groups D1 and D2 were less severe than that in group S,the severity of pruritus in the group D2 was less severe than that in the group D1,and the differences were statistically significant(P<0.05).Comparison of the severity of maternal nausea and vomiting between group D1 and group D2 showed no statistically significant difference(P>0.05).The incidence rates of nausea,vomiting and pruritus in groups D1 and D2 were lower than those in group S,the incidence of nausea,vomiting and pruritus in group D2 was lower than that in group D1,and the differences between the groups were statistically significant(P<0.05).Comparison of pain levels in the three groups within 24 h after receiving labor analgesia(except for 1 h of drug administration)showed no statistically significant difference(P>0.05).Comparison of Apgar scores at 1,5,and 10 minutes after birth and ICU admission rate at 7 days after birth among newborns in the three groups showed no statistically significant difference(P>0.05).Conclusion Intravenous pre-infusion of Nalmefene can significantly reduce the incidence and severity of nausea,vomiting and pruritus in epidural labor analgesia.Intravenous pre-infusion of 0.2 μg/kg of Nalmefene is better than that of 0.1 μg/kg and dose not affect analgesic effects of Sufentanil or infant outcomes.关键词
