基于α-SMA、TGF-β1、SphK1与S1P研究消痔灵注射治疗直肠黏膜内脱垂的机制
Mechanism of Xiaozhiling Injection in Treatment of Internal Rectal Prolapse Based on α-SMA,TGF-β1,SphK1 and S1P

Abstract
Objective To study the effect of Xiaozhiling and sterile water for injection on internal rectal prolapse(IRP),and to preliminarily explore the molecular mechanism of Xiaozhiling in the treatment of IRP.Methods The study was divided into two parts:animal experiment and cell experiment.In the an-imal experiment,the IRP rabbit model was first established and randomly divided into the Xiaozhiling group and the sterile water for injection group.Xiaozhiling injection and sterile water for injection were in-jected into the submucosal layer of the rectum respectively.After 2 weeks of normal feeding,the rectal tis-sues were dissected.The expressions of α-SMA,Col Ⅰ,Col Ⅲ,TGF-β1,SphK1 and S1PR1 were detected by immunohistochemistry.The protein expressions of α-SMA,TGF-β1,SphK1 and S1P were detected by Western blot.In the cell experiment,human fibroblasts were randomly divided into the control group,the 0.1%Xiaozhiling group and the 0.2%Xiaozhiling group according to different intervention methods.The control group was not added any solution,and the last two groups were added with the Xiaozhiling solu-tion.After 48 hours of intervention,the protein expressions of TGF-β1 and SphK1 in each group were de-tected by Wb method.Results In the animal experiments,obvious inflammatory infiltration and fibrotic granuloma formation were observed in the submucosa of the rabbit rectum in the Xiaozhiting group,and a little fibrosis formation was observed in the sterilized water group.Immunohistochemistry showed that the expressions of α-SMA,Col Ⅰ and Col Ⅲ in the rectal submucosa of the Xiaozhiling group were significant-ly higher than those of the sterilized water for injection group.Wb method showed that the protein expres-sions of α-SMA,TGF-β1,SphK1 and S1P in the rectal tissues of Xiaozhiling group were significantly higher than those of the sterilized water for injection group(P<0.05).Cell experiments showed that compared with the control group,the expressions of TGF-β1 and SphK1 in fibroblasts were up-regulated in the 0.1%Xiaozhiling group and 0.2%Xiaozhiling group(P<0.05),and the protein expressions of TGF-β1 and SphK1 in the 0.2%Xiaozhiling group were higher than those in the 0.1%Xiaozhiling group(P<0.05).Conclusion Xiaozhiling injection can significantly induce fibrosis formation by regulating the TGF-β1/SphK1-S1P signaling pathway,activating fibroblasts,up-regulating the expression of α-SMA,and promoting the synthesis,secretion and deposition of collagen fibers.关键词
