Abstract
Objective To compare the clinical efficacy of different doses of lysine hydrochloride and zinc gluconate combined with racecadotril in the treatment of rotavirusenteritis(RVE),and its effects on ser-um inflammatory factors and myocardial enzyme spectrum.Methods Total 96 children with RVE who were treated in our hospital from January 2021 to October 2024 were randomly divided into four groups,with 24 cases in each group.One group was given racecadotril granules(Sichuan Baili Pharmaceutical)orally(the control group),and the other 3 groups(collectively referred to as the combined treatment group)were given different doses of lysine hydrochloride and zinc gluconate granules(Hainan Huigu Phar-maceutical,5g per bag)orally on the basis of oral racecadotril granules,and were named the low-dose group,the medium-dose group and the high-dose group.All four groups were treated continuously for 7 days.The overall therapeutic effect,the time of antidiarrheal,antipyretic and antiemetic,the serum levels of interleukin(IL)-6 and tumor necrosis factor-α(TNF-α),and the serum levels of creatine kinase(CK),creatine kinase isoenzyme(CK-MB),lactate dehydrogenase(LDH)and aspartate aminotransferase(AST)in the four groups of children were compared.Results 1)Overall clinical efficacy:The total effective rates of the control group,the low-dose group,the medium-dose group and the high-dose group were 70.83％(17/24),79.17％(19/24),95.83％(23/24)and 91.67％(22/24),respectively.The total effec-tive rates of the three combined treatment groups were significantly higher than that of the control group(P＜0.05),which were higher in the medium-dose group and the high-dose group than in the low-dose group(P＜0.05),and there was no statistically significant difference between the medium-dose group and the high-dose group(P＞0.05).2)Time of antidiarrheal,antipyretic and antiemetic:The time of an-tidiarrheal,antipyretic and antiemetic in the three combined treatment group was significantly shorter than that of the control group(P＜0.05).The time of antidiarrheal and antipyretic in the medium-dose group and the high-dose group was shorter than that in the low-dose group(P＜0.05),and there was no statis-tically significant difference between the medium-dose group and the high-dose group(P＞0.05).The time of antiemetic in the medium-dose group was the shortest,which was longer in the high-dose group and the low-dose group than in the medium-dose group(P＜0.05),and there was no significant differ-ence between the high-dose group and the low-dose group(P＞0.05).3)Inflammatory factors:There was no statistically significant difference in the serum levels of IL-6 and TNF-α before treatment in the four groups(P＞0.05),and the serum levels of IL-6 and TNF-α in each group decreased after treatment(P＜0.05),which were the lowest in the high-dose group.4)Hematological indicators of myocardial damage:There was no statistically significant difference in the serum levels of CK,CK-MB,LDH and AST before treatment in the four groups(P＞0.05),and the serum levels of CK,CK-MB,LDH and AST in each group decreased after treatment(P＜0.05),which were the lowest in the medium-dose group and the high-dose group,but there was no significant difference between the two groups(P＞0.05).Conclusion Lysine hydrochloride and zinc gluconate combined with racecadotril is effective in the treatment of RVE.The medium-dose and high-dose have better efficacy and are similar in efficacy,but the high-dose has the risk of prolonging the vomiting time.It is recommended to use the medium-dose in clinical practice and not to blindly supplement zinc.关键词
