优化挂线疗法治疗复杂性肛瘘的系统评价与Meta分析
Systematic Review and Meta-Analysis of Optimized Thread-Drawing Therapy in Treating Complex Anal Fistula
摘要
Abstract
Objective To systematically evaluate the efficacy and safety of the optimized thread-drawing therapy in the treatment of complex anal fistula.Methods Chinese databases such as CNKI,Wanfang Data,CBM,and VIP were retrieved by computer to collect randomized controlled trial(RCTs)studies re-lated to the optimized thread-drawing therapy in the treatment of complex anal fistula.The search period was from the establishment of each database to June 2024.Two researchers independently searched and screened the literature.The quality of the literature that met the inclusion criteria was evaluated using the quality evaluation criteria provided by the Cochrane Manual of Systematic Review.Systematic review and Meta-analysis were conducted using RevMan 5.4 software,and sensitivity analysis was performed.Re-sults A total of 200 literatures were collected,and finally 15 RCTs studies were included,involving a to-tal of 1,140 patients with complex anal fistula.The results of Meta-analysis showed that the optimized thread-drawing therapy was superior to the traditional thread-drawing therapy in improving the treatment efficiency(P=0.0001),shortening the postoperative wound healing time(P＜0.00001),reducing the scar area(P＜0.00001),lowering the Wexner score of anal incontinence(P=0.001),decreasing the postoperative pain score(P＜0.00001),reducing the pain score on the 3rd postoperative day(P＜0.00001),and decreasing the pain score on the 7th postoperative day(P＜0.00001),and the differences were statistically significant.Conclusion The optimized thread-drawing therapy has advantages in en-hancing the treatment efficiency of complex anal fistula,improving the wound healing situation,protecting the anal function,and reducing postoperative pain,which is worthy of clinical promotion.关键词
