阿芬太尼联合喉罩通气对输尿管结石手术患者机体应激反应和血流动力学的影响
Effects of Alfentanil combined with Laryngeal Mask Airway Ventilation on Stress Response and Hemodynamics in Patients undergoing Ureterolithiasis Surgery

Abstract
Objective To explore the effects of alfentanil combined with laryngeal mask ventilation on the stress response and hemodynamics of patients undergoing ureteral calculi surgery.Methods A total of 95 patients with ureteral calculi who underwent surgical treatment at Hunan Provincial People's Hospital(the First Affiliated Hospital of Hunan Normal University)from June 2021 to March 2022 were selected and divided into the observation group(n=49)and the control group(n=46)according to different anesthesia regimens.The control group was given general anesthesia with propofol combined with sufentanil combined with laryngeal mask ventilation,while the observation group was given general anesthesia with propofol combined with alfentanil combined with laryngeal mask ventilation.The anesthetic effects of the two groups were compared.Results There was no significant difference in the comparison of the operation time,anesthesia recovery time,laryngeal mask removal time,intraoperative blood loss and atropine usage rate between the two groups(P＞0.05).At the two time periods before induction(T0)and 5 minutes after implantation(T2),there was no significant difference in heart rate(HR)and mean arterial pressure(MAP)between the two groups(P＞0.05).At the two time periods of immediately after the laryngeal mask was placed(T1)and immediately after the laryngeal mask was removed(T3),HR and MAP in both groups increased(P＜0.05),but those in the observation group were lower than those in the control group(P＜0.05).There was no significant difference in the Visual Analogue Scale(VAS)scores between the two groups 10 minutes after awakening and 24 hours after the operation(P＞0.05).During the two time periods of T0 and T2,there was no significant difference in the surgical volumetric index(SPI)between the two groups(P＞0.05).During the two time periods T1 and T3,the SPI of both groups increased(P＜0.05),but that of the observation group was lower than that of the control group(P＜0.05).The incidence of postoperative complications in the observation group was 16.33%,which was lower than 36.96%in the control group(P＜0.05).Conclusion In patients undergoing ureteral calculi surgery,the anesthesia regimen of alfentanil combined with laryngeal mask ventilation can better control the body's stress response and stabilize hemodynamics compared with the conventional sufentanil anesthesia regimen.Meanwhile,this regimen has good safety.关键词
