经导管主动脉瓣置换术治疗重度主动脉瓣狭窄的效果观察
Observation of the effect of transcatheter aortic valve replacement in the treatment of severe aortic stenosis

Abstract
Objective To explore the effect of transcatheter aortic valve replacement(TAVR)in the treatment of severe aortic stenosis(AS).Methods A total of 65 patients with severe AS admitted to the Department of Cardiovascular Surgery of the People's Hospital of Anyang City from February 2021 to February 2024 were selected.They were divided into the observation group(treated with TAVR,n=32)and the control group(treated with surgical aortic valve replacement[SAVR],n=33)according to the treatment plan.The perioperative conditions(operation time,hospitalization time,postoperative mechanical ventilation time,intraoperative hemorrhage volume),serum oxidative stress and inflammatory indices(malondialdehyde[MDA],superoxide dismutase[SOD],interleukin-1β[IL-1β],tumor necrosis factor-α[TNF-α]),and cardiac function indices(left ventricular ejection fraction[LVEF],left ventricular end-diastolic diameter[LVEDD],aortic valve maximum velocity[Vmax])were compared between the two groups.Results The operation time,hospitalization time,postoperative mechanical ventilation time,and intraoperative hemorrhage volume in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).At 24 hours after surgery,SOD was higher and MDA,IL-1β,and TNF-α in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).At 1 and 6 months after surgery,LVEF in the observation group was higher than that in the control group,and LVEDD and Vmax in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).Conclusion TAVR can effectively treat patients with severe AS,improve cardiac pump function and systemic blood circulation,simultaneously reduce trauma to the body,and improve the prognosis of AS patients.关键词
