分层激励约束机制在提高病原学送检率中的应用
The application of hierarchical incentive and constraint mechanism in improving etiological specimen submission rates

Abstract
Objective To investigate the impact and practical effectiveness of a management model combining hierarchical incentives and constraints on improving the submission of pathogenetic test samples before antimicrobial therapy.Methods The study implemented a hierarchical incentive and constraint mechanism to intervene in the submission of etiological specimens.Data were sourced from the real-time infection monitoring system of Liuzhou People's Hospital,focusing on hospitalized patients who received antimicrobial therapy between January 2022 and June 2024.The control group will be from January to December 2022,and the intervention group will be divided into two stages:the first stage will consist of 36 242 patients from January to December 2023,and the second stage will consist of 18 897 patients from January to June 2024.Changes in etiological submission indicators were compared before and after the intervention.Results Prior to the intervention,the submission rate of pathogenetic samples before the initial use of antimicrobial therapy was 36.51%,the targeted pathogenetic submission rate before the initial use of antimicrobial therapy was 36.10%,the pathogenetic submission rate related to hospital infection diagnosis was 58.80%,and the pathogenetic submission rate before the combined use of key antimicrobials was 93.17%.After the intervention,these submission rates increased to 76.08%,59.99%,96.99%,and 97.73%,respectively,all significantly higher than those of the control group,with statistically significant differences(P<0.001).Conclusion The implementation of a hierarchical incentive and constraint management mechanism can significantly improve the submission rate of pathogenetic samples before antimicrobial therapy,which is of great significance for enhancing the standardization and rationality of antimicrobial use,curbing the development of microbial resistance,and ensuring medical safety.关键词
