摘要
Abstract
Objective To investigate the related factors of Jarisch-Herxheimer reaction,and compare the effects of prednisone tablets and betamethasone injection in preventing Jarisch-Herxheimer reaction in syphilis treatment.Methods A total of 159 syphilis patients treated in the Department of Dermatology of Jilin City Hospital of Chemical Industry from December 2018 to October 2022 were selected as the research objects.Patients with syphilis were all treated with benzathine penicillin and randomly divided into the control group(n=77)and the treatment group(n=82).In the control group,77 cases were given oral prednisone tablets 30 mg/day for 3 continuous days before the first intramuscular injection of benzathine penicillin.In the treatment group,82 cases were treated with betamethasone injection.Among them,42 cases and 40 cases were randomly divided.Betamethasone 4 mg or 8 mg was given intravenously at the same time as the first benzathine penicillin treatment respectively.Compare the occurrence of the Gihai reaction between groups.Simultaneously,the related factors to Jarisch-Herxheimer reaction in different stages of syphilis and the titer of syphilis rapid plasma reagin test(RPR)were analyzed.Results The incidence of Jarisch-Herxheimer reaction was 10.39%in the control group,and the incidence of 4mg and 8 mg Jarisch-Herxheimer reaction in the treatment group was 16.67%and 10.00%,respectively,without statistically significant differences(P>0.05).In 159 cases of syphilis,patients with early syphilis and high syphilis RPR titer of syphilis were more likely to have Jarisch-Herxheimer reaction,and the symptoms appeared earlier.Conclusion When patients with early syphilis and patients with high RPR titer are treated for the first time,they should strengthen their awareness of preventing Jarisch-Herxheimer reaction and actively give preventive intervention.Simultaneously,glucocorticoid is effective in preventing Jarisch-Herxheimer reaction,which can greatly reduce the incidence and reaction degree.The preventive efficacy of a single dose of betamethasone(4 mg)is equivalent to that of continuous oral administration of prednisone tablets.This method is simple and easy,and it is worthy of clinical promotion and application.关键词
