Pharmacoeconomic Evaluation of Tribulus Teriflorus Saponin Capsules in the Treatment of Wind-phlegm and Blood Stasis Obstruction Syndrome during the Meridian Recovery Period of Stroke

Abstract
Objective To evaluate the economic viability of Tribulus terrestris saponin Capsules compared with Xinnao Shutong Capsules in the treatment of patients with wind-phlegm and blood stasis obstruction syndrome during the meridian recovery period of stroke.Methods Based on the phase Ⅲ clinical data and literature review of Tribulus terrestris saponin capsule,this study was carried out from the perspective of health system.Decision tree and Markov model were adopted to simulate the treatment cost and health outcomes of Tribulus terrestris saponin capsule and Xinnao Shutong capsule,and then the economy of the two treatment programs was evaluated.The robustness of the basic analysis results and the model were verified by single factor sensitivity analysis and probabilistic sensitivity analysis.Meanwhile,the results of a more recent phase Ⅳ clinical trial of Tribulus terrestris saponin capsule with a larger sample size were used for situational analysis.Results In the basic analysis,compared with Xinnao Shutong Capsules,the use of Tribulus terrestris saponin Capsules increased the treatment cost by 194.66 yuan in the short term(28 days),increased the health output by 0.003 QALYs,with an ICUR of 73 231.75 yuan/QALY,and reduced the treatment cost by 601.51 yuan in the long term(40 years).The health output increased by 0.13 QALYs,and the ICUR was-4 412.70 yuan/QALY,both of which were lower than the set WTP threshold.Therefore,it is considered that Tribulus terrestris saponin Capsules have better economy compared with Xinnao Shutong Capsules.The results of the sensitivity analysis show that the basic analysis results are relatively robust and will not reverse due to changes in model parameters.In the context analysis,both the short-term decision tree simulation results and the long-term Markov simulation results were lower than 1 times the per capita GDP in 2023.Based on the results of large-sample clinical trials,Tribulus teriphyllus saponin Capsules still have better economic performance compared to Xinnao Shutong Capsules.Conclusion Compared with Xinnao Shutong Capsules,Tribulus terrestris Saponin Capsules have pharmacoeconomic advantages in the treatment of wind-phlegm and blood stasis obstruction syndrome during the meridian recovery period of stroke.关键词
