经鼻高流量氧疗联合无创正压通气治疗对老年重症肺炎并呼吸衰竭患者的疗效分析

Abstract
Objective:To investigate the clinical effect of transnasal high flow oxygen therapy combined with non-invasive positive pressure ventilation in elderly patients with severe pneumonia and respiratory failure.Method:The data of 75 elderly patients with severe pneumonia and respiratory failure admitted to Beijing First Hospital of Integrated Traditional and Western Medicine from January 2019 to January 2023 were retrospectively analyzed.According to different oxygen therapy methods,they were divided into 37 cases in the control group(treated with non-invasive positive pressure ventilation)and 38 cases in the study group(treated with non-invasive positive pressure ventilation combined with nasal high flow oxygen therapy).The incidence of complications of blood gas index and respiratory circulation index before and after intervention was analyzed and compared between the two groups.Result:After 1 week of treatment,PaO2 in both groups was higher than that before treatment,and the difference was statistically significant(P<0.001).Covariance analysis results showed that after treatment,the oxygenation index(OI)of the study group was higher than that of the control group,the arterial blood pH value was lower than that of the control group,the heart rate was lower than that of the control group,and the complication rate was lower than that of the control group,with statistical significance(P<0.05).Conclusion:The combined application of nasal high-flow oxygen therapy and non-invasive positive pressure ventilation can effectively improve the clinical symptoms and lung function of elderly patients with severe pneumonia complicated with respiratory failure,and reduce the occurrence of complications.关键词
