PFNA内固定联合骨折端注入富血小板血浆对股骨粗隆间骨折患者血液流变学指标、骨折愈合及关节功能的影响
Effects of PFNA Internal Fixation Combined with Fracture-End Injection of Platelet-Rich Plasma on Hemorheology,Fracture Healing and Joint Function in Patients with Femoral Intertrochanteric Fracture

Abstract
Objective To explore the effects of proximal femoral nail anti-rotation(PFNA)internal fixation combined with fracture-end injection of platelet-rich plasma(PRP)on hemorheology,fracture healing,and joint function in patients with femoral intertrochanteric fractures.Methods In this retrospective study,108 patients with femoral intertrochanteric fracture were divided into the control group(n=52)and observation group(n=56)according to different surgical methods between June 2022 and June 2024.The control group was treated with PFNA internal fixation,while the observation group received addition-ally fracture-end injection of PRP.The perioperative indexes of the two groups were compared.Before treatment and at 1 week after surgery,hemorheology indexes[including whole blood high-shear viscosity,low-shear viscosity,plasma viscosity,erythro-cyte deformability index(TK),and erythrocyte rigidity index(IR)]were detected.At 1 month,3 months,and 6 months after sur-gery,fracture healing(assessed by Fernandez-Esteve callus score)and pain degree[using visual analogue scale(VAS)]were evaluated.At 6 months after surgery,the good rate of hip function was compared between the two groups,and the occurrence of postoperative complications was recorded.Results There was no significant difference in the length of surgical incision or op-eration time between the two groups(P＞0.05),but all other perioperative indexes in the observation group were better than those in the control group(P＜0.05).At different time points after surgery,whole blood high-shear viscosity,low-shear viscosity,plasma viscosity,IR,and VAS scores as well as the total incidence of postoperative complications in the observation group were lower than those in the control group,while TK,scores of Fernandez-Esteve callus,and good rate of hip function were higher than those in the control group(P＜0.05).Conclusion PFNA internal fixation combined with fracture-end injection of PRP can reduce intraoperative blood loss,shorten time to ambulation,hospital stay duration,and fracture healing time in patients with femoral intertrochanteric fractures.This therapeutic regimen can more effectively improve hemorheology indexes,promote fracture heal-ing,relieve pain,and improve hip function,with safe and reliable outcomes.关键词
