单髁置换与全膝关节置换治疗膝单间室骨关节炎的临床疗效及对步态和并发症的影响
Clinical Efficacy of Unicompartmental Knee Arthroplasty and Total Knee Arthroplasty in the Treatment of Unicompartmental Knee Osteoarthritis and Their Impacts on Gait and Complications

Abstract
Objective To explore the effects of unicompartmental knee arthroplasty(UKA)versus and total knee ar-throplasty(TKA)in patients with unicompartmental knee osteoarthritis,focusing on gait parameters and postoperative compli-cations.Methods This retrospective study included 186 patients with unicompartmental knee osteoarthritis treated at our hospital from August 2020 to April 2024.Patients were divided into the TKA group(n=88)and the UKA group(n=98)based on surgical approach.The following parameters were compared between groups:therapeutic outcomes(surgical time,intraopera-tive blood loss,time to ambulation,time to full weight-bearing),visual analogue scale(VAS)scores,knee function[assessed by Hospital for Special Surgery(HSS)score and maximum active knee flexion angle],gait parameters(stride length,stride frequen-cy,stride speed,support-phase-to—swing phase time ratio),complications,and Forgotten Joint Score(FJS-12).Results The UKA group demonstrated shorter time to full weight-bearing,earlier ambulation,reduced surgical time,and less intraoperative blood loss compared to the TKA group(P＜0.05).Postoperative VAS scores at 3 days,1 week,2 weeks,and 1 month were sig-nificantly lower in the UKA group(P＜0.05).At 6 months postoperatively,there was no significant difference in maximum active knee flexion angle between groups(P＞0.05).The HSS score was higher in the UKA group(P＜0.05).All gait parameters at 6 months favored the UKA group(P＜0.05).No significant difference was observed in total complication rates between groups(P＞0.05).The FJS-12 scores at 1,3,and 6 months were higher in the UKA group(P＜0.05).Conclusion Compared to TKA,UKA provides superior clinical outcomes,less postoperative pain,enhanced knee function recovery,improved gait parame-ters,and better proprioception restoration in patients with unicompartmental knee osteoarthritis.关键词
