The Predictive Value of Systemic Inflammatory Response Index for Post-ERCP Pancreatitis

Abstract
Objective To explore the predictive value of the systemic inflammatory response index(SIRI)for post-ER-CP pancreatitis.Methods From January 2020 to January 2024,39 patients who underwent ERCP treatment in Baoding Second Hospital and developed postoperative pancreatitis were classified as the concurrent group,and 81 patients who underwent ERCP treatment without postoperative pancreatitis were classified as the non-concurrent group.All subjects underwent neutrophil count,monocyte count,and lymphocyte count testing after admission,and SIRI was calculated.Demographic data and SIRI were compared between the groups.The predictive value of SIRI for post-ERCP pancreatitis was evaluated using ROC curve anal-ysis,and risk factors for post-ERCP pancreatitis were analyzed via multivariate logistic regression.Results The diameter of the common bile duct,ERCP time,platelet count,white blood cell count,neutrophil count,monocyte count,C-reactive protein,amylase,total bilirubin,creatinine,and SIRI in the pancreatitis group were significantly higher than those in the non-pancreati-tis group,while the lymphocyte count was lower(P＜0.05).Multivariate regression analysis showed that ERCP time(OR=2.421,95％CI:1.557-3.762),C-reactive protein(OR=3.678,95％CI:1.676-4.278),and SIRI(OR=4.384,95％CI:2.049-9.379)were all risk factors for post-ERCP pancreatitis(P＜0.05).ROC analysis showed that the area under the curve(AUC)of SIRI for pre-dicting post-ERCP pancreatitis was 0.909(95％CI:0.859-0.959).Conclusion SIRI is significantly elevated in patients with post-ERCP pancreatitis and can serve as a reliable predictor postoperative pancreatitis.关键词
