The Relationship between C-reactive Protein-Triglyceride Glucose Index and the Condition of Diabetic Foot Infection Patients and Its Prognostic Evaluation Value

Abstract
Objective To explore the relationship between C-reactive protein(CRP)—triglyceride glucose(TyG)index(CTI)and the condition of diabetic foot infection patients and its prognostic evaluation value.Methods A total of 152 diabetic foot infection patients admitted to our hospital from June 2021 to June 2024 were selected.They were divided into mild(n=57),moderate(n=63),and severe(n=32)groups based on the degree of infection at admission.Patients were also classified into the good prognosis(n=1 13)and the poor prognosis(n=39)groups based on their 6-month prognosis.The C-re-active protein(CRP),triglyceride-glucose(TyG)index and CTI at admission were compared among three groups of patients with different conditions and two groups of patients with different prognoses.The prognostic evaluation value of the above indicators for diabetic foot infection patients was analyzed by the ROC curves,and the influencing factors of prognosis in di-abetic foot infection patients were explored by multivariate logistic stepwise regression.Results As the severity of diabetic foot infection increased,CRP,TyG,and CTI gradually increased(P＜0.05).Spearman rank correlation analysis showed that CRP,TyG,and CTI were positively correlated with the severity of diabetic foot infection(r=0.512,0.437,0.605,respectively;P＜0.001).CRP,TyG,and CTI in the poor prognosis group were higher than those in the good prognosis group(P＜0.05).The AUC(95％CI)of CRP,TyG and CTI in evaluating the prognosis of patients with diabetic foot infection were 0.837(0.792-0.887),0.746(0.696-0.796),and 0.929(0.884-0.979),respectively.The AUC of CTI in evaluating the prognosis of cases with diabetic foot infection was higher than that of CRP and TyG(Z=12.374,P＜0.001;Z=13.258,P＜0.001).The levels of interleukin-6(IL-6),the proportion of Gram-negative bacteria detected,and the proportion of previous ulcer history in the poor prognosis group were higher than those in the good prognosis group,while the proportion of Gram-positive bacteria de-tected in the poor prognosis group was lower than that in the good prognosis group(P＜0.05).Previous ulcer history(OR=2.063,95％CI:1.106-3.847),CRP(OR=2.389,95％CI:1.264-4.518),and CTI(OR=3.691,95％CI:2.293-5.943)were the inde-pendent risk factors for prognosis in diabetic foot infection patients(P＜0.05).Conclusion CTI can reflect the severity of diabetic foot infection patients,and its increase is closely related to a poor prognosis of patients,providing an objective basis for clinical prognosis evaluation and stratified management.关键词
