龈沟液SICAM-1、RANKL在预测种植体周炎患者预后中的临床价值
Clinical Value of sICAM-1 and RANKL in Gingival Crevicular Fluid in Predicting the Prognosis of Patients with Peri-Implantitis

Abstract
Objective To investigate the clinical value of gingival crevicular fluid human soluble intercellular adhesion molecule-1(sICAM-1)and nuclear factor-κB receptor activator ligand(RANKL)in predicting the prognosis of peri-implan-titis(PI)patients.Methods Eighty-eight PI patients treated at our hospital from January 2020 to June 2024 were enrolled.Gingival crevicular fluid specimens were collected,and sICAM-1 and RANKL levels were measured via enzyme-linked immu-nosorbent assay(ELISA).Based on 6-month follow-up outcomes,PI patients were divided into the good prognosis group(n=63)and the poor prognosis group(n=25).Receiver operating characteristic(ROC)curves and area under the curve(AUC)were used to evaluate predictive performance,and multivariate logistic stepwise regression identified risk factors for the prognosis.Results The poor prognosis group had higher sICAM-1 and RANKL levels than the good prognosis group((P<0.05).ROC analysis re-vealed AUCs(95%CI)of 0.839(0.794-0.889)for sICAM-1,0.751(0.706-0.801)for RANKL,and 0.924(0.874-0.974)for their combination.The poor prognosis group had higher proportions of smokers,implants>13 mm,and history of periodontitis((P<0.05).Multivariate analysis identified periodontitis history(OR=2.208,95%CI:1.052-4.631),gingival crevicular fluid sI-CAM-1(OR=3.442,95%CI:1.908-6.209),and RANKL(OR=4.437,95%CI:2.132-9.235)as independent risk factors(P<0.05).Conclusion The levels of sICAM-1 and RANKL are significantly elevated in PI patients with poor prognosis.The combined detection of the two can help predict the prognosis of PI patients.关键词
