The Relationship between Serum CCR2 and CCL2 Levels and the Condition and Prognosis of Patients with IgA Nephropathy

Abstract
Objective To investigate the relationship between serum levels of CC chemokine receptor 2(CCR2)and CC chemokine ligand 2(CCL2)in patients with IgA nephropathy and their association with disease and prognosis.Methods A total of 106 patients with IgA nephropathy admitted to Guangyuan Central Hospital from January 2020 to December 2022 were selected as the study group,and 100 healthy individuals who underwent physical examinations were selected as the control group.Patients were divided into a mild group(n=42),a moderate group(n=35),and a severe group(n=29)based on renal inter-stitial fibrosis severity.After a two-year follow-up,the patients were classified into a good prognosis group(n=78,73.58%)and a poor prognosis group(n=28,26.42%.Clinical data and serum CCR2/CCL2 levels at admission were analyzed for correlations with disease severity.Multivariate logistic regression was used to identify risk factors for poor prognosis.Results Serum CCR2 and CCL2 were significantly elevated in the study group compared to controls(P<0.05)and increased progressively with disease severity(P<0.05).Spearman analysis revealed strong positive correlations between serum CCR2(r=0.613)and CCL2(r=0.684)levels and disease severity(P<0.05).At follow-up,the poor prognosis group had significantly higher 24-hour urine protein,serum CCR2,serum CCL2,blood urea nitrogen,serum creatinine,and uric acid,along with lower eGFR,compared to the good prognosis group(P<0.05).Multivariate logistic regression analysis identified serum creatinine(OR=2.277,95%CI:1.048-4.949),24-hour urine protein(OR=2.502,95%CI:1.226-5.106),serum CCR2(OR=3.093,95%CI:1.711-5.590),and serum CCL2(OR=3.800,95%CI:2.066-6.991)as independent risk factors for poor prognosis(P<0.05).Conclusion Serum CCR2 and CCL2 levels are strongly associated with disease severity and prognosis in IgA nephropathy,suggesting their potential as bio-markers for disease monitoring and outcome prediction.关键词
