Influence of PRF on Pain,Periodontal Tissue Changes and Tooth Movement after Orthodontic Extraction

Abstract
Objective To investigate the influence of platelet-rich fibrin(PRF)on pain,periodontal tissue changes and tooth movement after orthodontic tooth extraction.Methods From June 2023 to September 2024,120 malocclusion patients undergoing orthodontic extraction at Harrison International Peace Hospital were randomized into control(n=60)and observation(n=60)groups.Post-extraction,control sockets were cleaned and filled with natural clots,while observation sockets received PRF gel and film derived from peripheral venous blood with tension-free sutures.Buccal keratinized gingival height(preoper-atively,14 d,and 3 mo postoperatively),tooth movement distance,gingival crease(1,2,and 3 mo postoperatively),pain status(VAS at 1 d,3 d,and 7 d postoperatively),and healing status were compared between groups.Postoperative complications were recorded.Results Before and at 14 d/3 mo post-surgery,buccal keratinized gingival height in the control group first decreased then increased,with significant intra-group differences(P<0.05),while no significant changes occurred in the observation group(P>0.05).The observation group showed significantly higher gingival height than the control group at 14 d(P<0.05).At 1,2,and 3 mo post-surgery,buccal and mesial-distal tooth movement distances progressively increased in both groups(P<0.05),with longer distances in the observation group(P<0.05).Vertical and parallel probing depths increased over time in both groups(P<0.05),but were significantly lower in the observation group(P<0.05).The observation group had a lower cumulative gingival crease incidence at 3 mo(P<0.05).VAS scores gradually decreased post-surgery in both groups(P<0.05),with lower scores in the observation group(P<0.05).Conclusion PRF application in extraction sockets after orthodontic extraction effectively relieves the postoperative pain,promotes the recovery of buccal keratinized gingival height,accelerates the tooth movement,re-duces the occurrence of gingival crease,and demonstrates good safety.关键词
