Abstract
Objective To investigate the effect of micro—flap alveolar ridge preservation on the therapeutic efficacy of dental implant in patients with severe bone destruction after tooth extraction.Methods A total of 104 patients with severe periodontal disease and severe bone destruction who underwent mandibular molar extraction and planned to receive implant restoration in our hospital from June 2020 to January 2023 were randomly divided into an experimental group and a control group,with 52 patients in each group.The experimental group underwent minimally invasive tooth extraction combined with micro-flap alveolar ridge preservation surgery,while the control group underwent minimally invasive tooth extraction alone.Follow up assessments were conducted before tooth extraction(T0),immediately after tooth extraction(T1),6 months after tooth extraction(T2),and 6 months after implant implantation(T3)on the day of permanent restoration(T4),and 6 months after per-manent restoration(T5).Clinical and imaging examinations were performed at T5 to evaluate alveolar bone height,alveolar bone width,keratinized gingival width,and periodontal condition.Results At T2,the increase in bone height in the experimental group was greater than that in the control group(P<0.05);At T2,the reduction in alveolar ridge width at the 1/4,1/2,and 3/4 root length sites in the experimental group was less than that in the control group(P<0.05).At T4 and T5,the keratinized gin-gival width of the experimental group decreased compared to T0,but the difference was not statistically significant(P>0.05);however,the keratinized gingival width of the control group was significantly reduced compared to T0(P<0.05);The keratinized gingival width in the experimental group was greater than that in the control group(P<0.05).At T5,the periodontal condition of both groups improved significantly compared to T0(P<0.05),and the plaque index,gingival index,and gingival bleeding index of the experimental group were all lower than those of the control group(P<0.05).Conclusion For patients with severe periodontitis and severe bone destruction who undergo implant restoration,micro-flap ridge preservation can help preserve the width,height,and keratinized gingival width of the implant site,leading to more ideal postoperative periodontal conditions.关键词
