Abstract
Objective To investigate the effect of the mean platelet volume to count ratio(MPV/PLT)on the prognosis of patients with colorectal cancer after radical surgery.Methods A total of 102 patients undergoing radical resection for colorec-tal cancer at Hai'an People's Hospital of Jiangsu Province from January 2019 to January 2021 were included.The optimal cut-off value of preoperative MPV/PLT was determined using receiver operating characteristics(ROC)curve analysis;patients were subsequently divided into high-and low-MPV/PLT groups based on this value.Associations between preoperative MPV/PLT and both clinical data and prognosis were analyzed.Results The ROC curve showed an optimal cut-off value of MPV/PLT was 0.038,with a curve area of 0.773(95％CI:0.728-0.823),sensitivity was 85.07％,and specificity of 58.29％.Based on this threshold,patients were stratified into high MPV/PLT group(＞0.038,n=59)and low MPV/PLT group(≤0.038,n=43).No sig-nificant differences were observed in gender,age,BMI,tumor location,differentiation degree,monthly family income,smoking history,or alcohol consumption between groups(P＞0.05).Significant differences emerged in albumin levels,fibrinogen levels,TNM stage,tumor length,postoperative adjuvant chemoradiation,and neurovascular invasion(P＜0.05).Kaplan-Meier revealed an overall survival(OS)range of 8-48 months post-surgery,with a median survival of 18 months.The high MPV/PLT group demonstrated longer median OS(24 vs 12 months;Log-Rank x2=12.582,df=1,P＜0.05).Univariate analysis identified TNM stage,albumin,fibrinogen,neurovascular invasion,and MPV/PLT as prognostic factors(P＜0.05).Multivariate Cox regression analysis showed TNM stage Ⅲ(HR=2.380,95％CI:1.327-4.268),neurovascular invasion(HR=2.591,95％CI:1.428-4.701),and MPV/PLT≤0.038(HR=3.777,95％CI:1.808-7.893)as independent risk factors(P＜0.05).Conclusion Decreased pre-operative MPV/PLT is a risk factor for poor prognosis in colorectal cancer patients after radical resection,demonstrating clinical significance for postoperative prognostic assessment.关键词
