熄风化痰醒窍汤联合左乙拉西坦对缺血性卒中后继发癫痫患者受损神经元恢复的影响
The Effect of Xifeng Huatan Xingqiao Decoction Combined with Levetiracetam on the Recovery of Damaged Neurons in Patients with Secondary Epilepsy After Ischemic Stroke

Abstract
Objective:To observe the effect of Xifeng Huatan Xingqiao Decoction combined with levetiracetam on the recovery of damaged neurons in patients with secondary epilepsy after ischemic stroke.Methods:A total of 116 patients with secondary epilepsy after stroke,diagnosed with wind-phlegm obstruction syndrome according to traditional Chinese medicine(TCM)criteria,were selected from the Department of Encephalopathy at Ankang Hospital of Traditional Chinese Medicine from June 2021 to December 2023.The patients were divided into a control group(58 enrolled,57 completed data collection)and an observation group(58 enrolled,57 completed data collection)using a simple randomization method.Both groups received conventional symptomatic treatment.The control group was additionally treated with levetiracetam,while the observation group received Xifeng Huatan Xingqiao Decoction combined with levetiracetam.The clinical total effective rate,changes in neurotrophic factors,inflammatory mediators,and electroencephalogram(EEG)were measured.The differences in TCM syndrome scores and the incidence of adverse reactions were also evaluated.Results:The total effective rate in the observation group was 92.98%(53/57),significantly higher than that in the control group(78.95%,45/57)(P<0.05).After treatment,brain-derived neurotrophic factor(BDNF)levels increased in both groups(P<0.05),while matrix metalloproteinase-9(MMP-9),interleukin-2(IL-2),neuron-specific enolase(NSE),interleukin-1β(IL-1β),S100 calcium-binding protein β(S100β),tumor necrosis factor-α(TNF-α),and interleukin-8(IL-8)levels decreased(P<0.05).The observation group showed higher BDNF levels and lower NSE,S100β,and inflammatory mediator levels compared to the control group(P<0.05).The number of epileptiform discharges and involved leads on EEG decreased in both groups after treatment(P<0.05),with the observation group showing greater improvement than the control group(P<0.05).TCM syndrome scores,including symptoms such as unconsciousness,limb convulsions,stiffness,drooling,dizziness,chest tightness,fatigue,and excessive phlegm,were lower in both groups after treatment(P<0.05),with the observation group showing greater improvement(P<0.05).The incidence of adverse reactions was 10.53%(6/57)in the observation group and 7.02%(4/57)in the control group,with no significant difference between the two groups(P>0.05).Conclusion:The combination of Xifeng Huatan Xingqiao Decoction and levetiracetam can repair damaged neurons,reduce the release of inflammatory mediators,decrease epileptic seizures,and improve clinical efficacy in patients with secondary epilepsy after stroke.关键词
