三孔法与四孔法腹腔镜联合胆道镜胆总管探查术治疗胆总管结石的临床效果比较
Comparison of three-port and four-port laparoscopic-choledochoscopic common bile duct exploration in the treatment of choledocholithiasis

Abstract
Objective To compare the clinical application value of three-port and four-port laparoscopic-choledochoscopic common bile duct exploration in the treatment of choledocholithiasis.Methods A retrospective analysis was conducted on 107 patients with choledocholithiasis,who underwent laparoscopic-choledochoscopic common bile duct exploration between Feb.2019 and Dec.2022 in the 901th Hospital,the Joint Logistics Support Force of the People's Liberation Army of China.Patients were divided into the three-port laparoscopic group(n=52)and the four-port laparoscopic group(n=57).The intraoperative and postoperative outcomes were compared between the two groups.Results Laparoscopic-choledochoscopic common bile duct exploration was successfully completed in all patients,with no case of three-port laparoscopic surgery group conversion to four-port laparoscopic surgery,with no case of conversion to open surgery in both groups.There were no statistically significant differences in terms of operation time,intraoperative bleeding,the rate of tetaining T tube,total hospitalization costs,postoperative liver function and inflammatory indicators,or the incidence of complications between the two groups(all P>0.05).However,patients in the three-port laparoscopic group had longer postoperative exhaust time[(2.3±1.4)min vs(1.8±0.6)min,P=0.025]and postoperative hospitalization time[(7.3±3.1)d vs(6.2±2.1)d,P=0.035],the difference were statistically significant.Conclusion For the treatment of choledocholithiasis,three-port laparoscopic-choledochoscopic common bile duct exploration are equivalent to four-port surgery.关键词
