针刺联合中药熏洗治疗糖尿病周围神经病变的疗效观察及对血液流变学的影响
Therapeutic observation of acupuncture plus Chinese medicine steaming and washing for diabetic peripheral neuropathy and the effect on hemorheology

Abstract
Objective To observe the clinical efficacy of acupuncture plus Chinese medicine steaming and washing in treating diabetic peripheral neuropathy(DPN)and the effect on the patient's hemorheological indicators.Method A total of 120 DPN patients were divided into a Western medicine group,a Chinese medicine group,and a joint treatment group using the random number table method,with 40 cases in each group.The Western medicine group was treated with basic symptomatic Western medications;the Chinese medicine group received Chinese medicine steaming and washing treatment in addition to the intervention in the Western medicine group;the joint treatment group was intervened by acupuncture at the eight confluent points in addition to the intervention given to the Chinese medicine group.The clinical efficacy and adverse reaction rate were compared across the three groups.The three groups were observed before and after the treatment for changes in the pain visual analog scale(VAS)score,electromyographic(EMG)parameters[motor nerve conduction velocity(MCV)and sensory nerve conduction velocity(SCV)of the dominant limb],blood glucose indicators[glycosylated hemoglobin(HbA1c),fasting plasma glucose(FPG),and 2-hour postprandial blood glucose(2hPG)],hemorheological indicators[plasma viscosity,whole blood high shear viscosity,whole blood medium shear viscosity,and whole blood low shear viscosity],and Toronto clinical scoring system(TCSS)score.Result The joint treatment group achieved a higher total effective rate(92.5%)than the Western medicine group(70.0%)and the Chinese medicine group(75.0%),and the differences were statistically significant(P＜0.05).After the treatment,the VAS score and the levels of blood glucose indicators,hemorheological parameters,and TCSS dropped in all three groups(P＜0.05)and were lower in the joint treatment group than in the other two groups(P＜0.05),and the Chinese medicine group was lower than the Western medicine group(P＜0.05).After the intervention,the EMG readings showed an increase in the fibular MCV and SCV of the dominant limb in all three groups;the joint treatment group was higher than the Western medicine and Chinese medicine groups(P＜0.05),and the Chinese medicine group was higher than the Western medicine group(P＜0.05).The three groups were statistically equal when comparing the adverse reaction rate(P＞0.05).Conclusion Incorporating acupuncture and Chinese medicine steaming and washing into the basic symptomatic Western medicine treatment can produce more significant clinical efficacy than the sole use of either Western medication or Chinese medicine steaming and washing in treating DPN;this method can relieve pain,elevate EMG parameter levels,strengthen the patient's muscle function,and improve their hemorheology and blood glucose levels.关键词
