超声下针刺后溪穴治疗椎动脉型颈椎病的疗效观察
Clinical observation on ultrasound-guided acupuncture at Houxi(SI3)for vertebral artery type cervical spondylosis

Abstract
Objective To observe the clinical efficacy of ultrasound-guided acupuncture at Houxi(SI3)in treating vertebral artery type cervical spondylosis(VATCS).Method Sixty patients were randomly divided into an observation group and a control group at a 1:1 ratio,with 30 cases in each group.Both groups received ultrasound-guided acupuncture.The observation group was needled at the affected-side Houxi until arrival of Qi was achieved,while the control group was needled 1 cm lateral to Houxi without arrival of Qi.Clinical efficacy was compared between the two groups.The visual analog scale(VAS)score,functional assessment scale for VATCS(FS-CSA)score,mean velocity(Vm)and pulsatility index(PI)of the vertebrobasilar artery,bilateral maximum temperature difference in the neck and occiput,and the clinical evaluation scale score for needle sensation were observed before and after treatment.Ultrasound anatomical images of Houxi were collected,and safety was assessed in both groups.Result After treatment,the total effective rates in the observation and control groups were 70.0%and 33.3%,respectively,with a statistically significant difference(P＜0.05).In the observation group,the VAS score,FS-CSA score,vertebrobasilar PI,and bilateral maximum temperature difference in the neck and occiput decreased(P＜0.05),while the vertebrobasilar Vm increased(P＜0.05).The observation group showed better improvements in VAS score,FS-CSA score,vertebrobasilar Vm and PI,and bilateral maximum temperature difference compared to the control group(P＜0.05).Ultrasound anatomical analysis revealed that the needle insertion depth in the observation group was greater than that in the control group(P＜0.05).Both groups maintained good blinding,and no significant adverse reactions occurred.Conclusion Ultrasound-guided acupuncture at Houxi with arrival of Qi can alleviate pain,improve symptoms and local blood supply in VATCS,demonstrating certain clinical efficacy.关键词
