喉癌术后病人匹配式吞咽训练方案的构建与应用
Construction and application of matched swallowing training program for postoperative patients with laryngeal cancer

Abstract
Objective:To construct a matched swallowing training program for postoperative patients with laryngeal cancer,and to explore its application effect.Methods:Through evidence summary and expert meeting methods,the matched swallowing training program for postoperative patients with laryngeal cancer based on the score of the Standardized Swallowing Assessment(SSA)and surgical methods was constructed.A total of 71 patients with laryngeal cancer who were admitted to the head and neck surgery department of a tertiary grade A hospital in Shanxi province from May to October 2023 were selected,and randomly divided into intervention group of 35 cases and control group of 36 cases.The control group received routine perioperative care and routine rehabilitation training for laryngeal cancer.The intervention group adopted the constructed matched swallowing training program for postoperative patients with laryngeal cancer.The swallowing function,indwelling time of gastric tube,quality of life related to swallowing,the incidence of aspiration and complications were compared between the two groups of patients.Results:There were statistically significant differences in terms of the intergroup effect,time effect and interaction effect of the swallowing function scores between the two groups of patients(all P<0.01).After 4 weeks of intervention,the swallowing function score of the intervention group was lower than that of the control group(P<0.01).There were statistically significant differences in terms of the intergroup effect,time effect and interaction effect of the quality of life related to swallowing between the two groups of patients(all P<0.01).After 2 and 4 weeks of intervention,the scores of quality of life related to swallowing in the intervention group were higher than those in the control group(P<0.01).The indwelling time of gastric tubes in the intervention group was shorter than that in the control group(P<0.01).There were no statistically significant difference in terms of the incidence of aspiration and complications between the two groups of patients(P>0.05).Conclusions:The matched swallowing training program for postoperative patients with laryngeal cancer based on SSA score and surgical methods was scientific and practical.It could effectively improve the early swallowing function and the quality of life related to swallowing of patients,shorten the indwelling time of gastric tubes.And it may provide a reference for the swallowing rehabilitation management of patients with laryngeal cancer.关键词
