Abstract
Objective:To analyze the clinical characteristics and pathogen distribution of infections in patients with severe traumatic brain injury(TBI),and to investigate factors associated with pulmonary infection.Methods:A retrospective cohort study was conducted on 329 severe TBI patients admitted to the intensive care unit of the First Affiliated Hospital of Shantou University Medical College from January 2021 to May 2024.Clinical data were collected to analyze infection types and pathogen profiles.Multivariate logistic regression was used to identify factors associated with pulmonary infection.Results:The overall infection rate was 50.5%(166/329),including 137 cases(41.6%)of single infection,predominantly pulmonary infection(36.8%,121/329),and 29 cases(8.8%)of mixed infection,most commonly pulmonary combined with intracranial infection(3.3%,11/329).A total of 220 pathogenic strains were isolated,with Gram-negative bacteria accounting for 71.8%(158/220),primarily Klebsiella pneumoniae(21.4%)and Pseudomonas aeruginosa(20.9%).Gram-positive bacteria constituted 17.3%(38/220),mainly Staphylococcus aureus(10.9%),while fungi accounted for 10.9%(24/220),predominantly Candida albicans(7.3%).Multivariate logistic regression analysis revealed that tracheotomy(OR=4.129,95%CI:2.255-7.561),feeding intolerance(OR=2.060,95%CI:1.186-3.577),and duration of mechanical ventilation(OR=1.002,95%CI:1.001-1.004)were significantly associated with pulmonary infection in severe TBI patients(all P<0.05).Conclusion:Infections in severe TBI patients are predominantly caused by Gram-negative bacteria,with pulmonary infection being the most common.Tracheotomy,feeding intolerance and prolonged mechanical ventilation may increase the risk of pulmonary infection in severe TBI patients.关键词
