Abstract

Objective:To systematically evaluate gender differences in thrombosis among individuals aged≥60 years following COVID-19 vaccination.Methods:A computerized search was conducted in the PubMed database to identify literature reporting thrombosis post-COVID-19 vaccination.Studies meeting predefined inclusion and exclusion criteria were analyzed.Results:A total of 117 elderly patients with post-vaccination thrombosis were included,comprising 58 males(median age:66[interquartile range(IQR):62-75]years)and 59 females(median age:68[IQR:62-75]years),with no statistically significant differences in age distribution(P>0.05).Males exhibited a higher prevalence of diabetes than females(36.2%vs.17.6%,P=0.030).The ChAdOx1 nCoV-19 vaccine was the most commonly administered type,with a higher proportion of females receiving this vaccine compared to males(47.5%vs.29.3%,P=0.044).Females more frequently presented with gastrointestinal symptoms(27.6%vs.8.9%,P=0.010)and had a higher incidence of multi-site thrombosis(40.7%vs.17.2%,P=0.005).No gender-based differences were observed in clinical management strategies(e.g.,anticoagulation,immunomodulatory therapy)or outcomes(mortality,complication rates).Conclusion:Enhanced monitoring for thrombotic events should be prioritized in elderly female populations following COVID-19 vaccination.
