Abstract
Objective To analyze the influencing factors of persistent human papilloma virus(HPV)infection in patients with high-grade squamous intraepithelial lesion(HSIL)after surgery.Methods A total of 100 patients diagnosed with HSIL and HPV infection in the Department of Gynecology,the First People's Hospital of Wusu District in Longnan from January 2021 to January 2023 were selected.According to whether HPV turned negative one year after surgery,they were divided into HPV persistent infection group(n=32)and HPV turned negative group(n=68).The clinical data of patients'age,preoperative viral load,postoperative pathological resection margin,postoperative use of anti HPV infection drugs,vaginal microecology were collected.Univariate and multivariate logistic regression were used to analyze the influencing factors of persistent HPV infection after HSIL.Results Univariate analysis showed that age,preoperative viral load,postoperative pathological resection margin,vaginal microecology(vaginal microbiota diversity,dysbiosis of flora status,and flora density)were associated with postoperative persistent HPV infection in patients with HSIL(P<0.05).Multivariate analysis showed that age>60 years old,preoperative viral load≥100 copies/ml,positive of postoperative pathological resection margin,abnormal flora density,dysbiosis and abnormal flora diversity were independent risk factors for postoperative HPV persistent infection in patients with HSIL(P<0.05).Conclusion Age,preoperative viral load,postoperative pathological resection margin and vaginal microecology are independent risk factors for HPV persistent infection in patients with HSIL after surgery,which should be paid attention in clinical practice,and relevant measures should be formulated to prevent and intervene HPV persistent infection.关键词
