Abstract
Objective To explore the relationship between serum levels of smooth muscle 22α(SM22α)and Elabela with the degree of carotid artery stenosis and cognitive function in patients with acute cerebral infarction(ACI).Methods A total of 145 ACI patients treated in the Affiliated Hospital of Hubei Medical College from November 2021 to November 2023 were selected as ACI group,and 145 healthy people were selected as control group during the same period.Patients with ACI were divided into mild,moderate,and severe groups according to the degree of carotid artery stenosis,and into cognitive normal and cognitive impairment groups according to the Mini-Mental State Examination(MMSE)scores.Enzyme linked immunosorbent assay was used to determine the expression levels of serum SM22α and Elabela.Spearman method was applied to analyze the correlation between serum SM22α,Elabela,degree of carotid artery stenosis,and cognitive function.Multivariate Logistic regression was applied to analyze the factors that affected cognitive impairment.The diagnostic efficacy of SM22α and Elabela on cognitive impairment was analyzed by drawing the receiver operating characteristic(ROC)curve.Results Compared with the control group,the expression levels of serum SM22α and Elabela in the ACI group decreased(t=7.818,11.311;P<0.05).Serum SM22α and Elabela expression levels in the mild,moderate and severe groups decreased sequentially(P<0.05).Compared with the cognitive normal group,the expression levels of serum SM22α and Elabela in the cognitive impairment group decreased(t=9.255,7.420;P<0.05).The serum levels of SM22α and Elabela in ACI patients were negatively correlated with the degree of carotid artery stenosis(r=-0.562,-0.624;P<0.05),and negatively correlated with cognitive impairment(r=-0.408,-0.435;P<0.05).Multivariate Logistic regression analysis showed that SM22α and Elabela were protective factors of cognitive impairment(OR=0.878,0.764;P<0.05),and hypertension was a risk factor of cognitive impairment(OR=3.025,P<0.05).The combined diagnosis of serum SM22α and Elabela had the highest AUC for cognitive impairment,which was better than the individual diagnosis of serum SM22α and Elabela(Z=3.406,2.767;P<0.05),with a sensitivity of 94.03%and a specificity of 76.92%.Conclusion The expression levels of serum SM22α and Elabela in ACI patients are reduced,both of them are related to the degree of carotid artery stenosis and cognitive function.关键词
