摘要

Abstract

Biventricular pacing cardiac resynchronization therapy(BVP-CRT)is considered the primary treatment for patients with heart failure combined with reduced left ventricular ejection fraction(LVEF)and electro-mechanical dyssynchrony of the heart.However,part of patients receiving BVP-CRT do not respond to the treatment.Left bundle branch area pacing(LBBaP)is a new method emerging in recent years,which can improve the electrical activity and pumping function of the heart by stimulating the left bundle branch region.Multiple studies have shown that LBBaP is superior to BVP.It can provide CRT with a narrower QRS duration,and it leads to greater improvements in LVEF and cardiac function.However,LBBaP is not suitable for all heart failure patients who require CRT,and its success rate in heart failure patients is limited due to myocardial fibrosis,nonspecific intraventricular conduction delay,and right atrium or right ventricle enlargement.This article reviews the advantages and challenges of LBBaP in CRT treatment of heart failure.
