摘要

Abstract

Objective:To explore the value of proton magnetic resonance spectroscopy(1H-MRS)in guiding the pathological grading of brain glioma(BG).Methods:The data of 112 patients with BG at our hospital were retrospectively collected between April 2021 and April 2024.According to grading criteria of BG,patients were divided into high-grade BG group(n=69)and low-grade BG group(n=43).All patients underwent 1H-MRS examination following admission.The metabolite ratios of BG in different areas,and metabolite ratios in the core parenchyma area of the tumor with different grading of BG were compared.The differentiation value of metabolite parameters in pathological grading of BG was analyzed.Results:Compared with contralateral normal white matter area,Cho/Cr and Cho/NAA values were higher,while NAA/Cr value was lower in the core parenchyma area of the tumor(P<0.05).There were 62 cases with enhancement,43 cases with necrosis,49 cases with hemorrhage and 52 cases with calcification in high-grade BG group,more than those in low-grade BG group(3 cases with enhancement,5 cases with necrosis,8 cases with hemorrhage,14 cases with calcification),and the difference was statistically significant(P<0.05).The difference in tumor diameter between high-grade BG group and low-grade BG group was not statistically significant((5.42±0.75)cm vs.(5.38±0.63)cm,P>0.05).Compared with low-grade BG group,Cho/Cr and Cho/NAA were higher in high-grade BG group(P<0.05).The results of ROC curves analysis showed that AUC of Cho/Cr combined with Cho/NAA in the differential diagnosis of BG pathological grading was 0.946,greater than that of single index(P<0.05).Conclusion:The metabolite indexes detected by 1H-MRS can reflect tumor metabolites in BG patients,which can provide guidance for pathological grading.
