肺癌肿瘤脉管浸润及浸润转移的CT征象研究
Research on CT signs of tumor vascular invasion and metastasis in lung cancer

Abstract
Objective:To investigate the CT signs of tumor vascular invasion and metastasis in lung cancer.Methods:A total of 116 patients with lung cancer in our hospital were selected,including 61 patients with vascular invasion,55 patients with non-vascular invasion,57 patients with invasive metastasis and 59 patients with non-invasive metastasis.The CT signs of patients in the vascular invasion group and the non-vascular invasion group,and the CT signs in the invasive metastasis group and the non-invasive metastasis group were compared.The correlation between CT signs and tumor vascular invasion and metastasis was analyzed by Spearman.The diagnostic efficacy of CT examination for tumor vascular invasion and metastasis in patients with lung cancer was analyzed by ROC curve.Results:There were significant differences in CT signs between the vascular invasion group and the non-vascular invasion group in tumor size,margin,boundary,adjacent structures,and pleural depression(P<0.05).There were significant differences in CT signs between the invasion and metastasis group and the non-invasion and metastasis group in tumor size,margin,boundary,adjacent structures,and pleural depression(P<0.05).The tumor volume in the vascular invasion group and the invasion and metastasis group was large,with blurred edge and disordered surrounding tissue structure,often accompanied by obvious vascular shadow.Spearman's correlation analysis showed that the tumor margin,adjacent structures,and relationship with the pleura in CT signs were negatively correlated with tumor vascular invasion and metastasis(P<0.05),and tumor size,the minimum distance between the lesion and pleura were positively correlated with tumor vascular invasion and metastasis(P<0.05).The results of ROC curve analysis showed that the tumor margin,the relationship between adjacent structures and pleura,pleural depression,tumor size,and the minimum distance between lesions and pleura in CT signs had high diagnostic efficacy for vascular invasion and invasion and metastasis.The AUC values were 0.714,0.746,0.639,0.772,0.925,0.955 and 0.665,0.681,0.645,0.9,0.835,0.873,respectively(P<0.05).Conclusion:CT examination has a higher diagnostic value in the diagnosis of vascular invasion and tumor invasion metastasis in lung cancer.Certain CT features,such as irregular tumor margins,burr signs,vascular convergence signs,air bronchograms,and pleural traction signs,can serve as effective indicators for assessing vascular invasion and invasion and metastasis,aiding clinicians in the diagnosis and treatment of lung cancer.关键词
