Abstract
Objective:To explore the value of spectral CT quantitative parameters in assessing the pathological grading of lung adenocarcinoma.Methods:We retrospectively analyzed clinical and imaging data from 77 patients with lung adenocarci-noma confirmed by surgical pathology.According to the grading system established by the International Association for the Study of Lung Cancer in 2020,all cases were divided into low-grade(G1 and G2)and high-grade(G3)groups.The spectral CT quantitative parameters in both the arterial and venous phases of the lesions were assessed,including CT values at 70 keV and 40 keV,standardized iodine concentration,and spectral curve slope(K).Differences in clinical and CT features and spec-tral CT quantitative parameters were compared between the two groups.To build a diagnostic model for predicting the patho-logical grading of lung adenocarcinoma,the variables with statistically significant differences were included into the multivariate Logistic regression analysis.The diagnostic performance of the model was assessed using receiver operating characteristic curve and area under the curve(AUC).Results:Between the low-grade and high-grade groups,tumor diameter,nodule density,and K value in both the arterial and venous phases were statistically significant differences.The AUC of the clinical model,pa-rameter model and combined model constructed based on multivariate Logistic regression analysis were 0.692,0.697 and 0.752,respectively.The diagnostic efficacy of the combined model was better than that of the clinical model and parameter model.Conclusion:Spectral CT quantitative parameters have potential application value in assessing the pathological grade of lung adenocarcinoma.The combined model based on CT features and quantitative parameters is helpful for preoperative differ-entiation of the pathological grade of lung adenocarcinoma.关键词
