Abstract
Objective:To analyze the predictive value of CT for lymph node metastasis in colorectal adenocarcinoma.Methods:A total of 150 patients admitted to the hospital from January 2023 to August 2024 due to colorectal adenocarcinoma were selected.All patients underwent enhanced CT scan of the whole abdomen before surgery,and their CT image features were collected.Based on postoperative pathological results,patients enrolled were divided into the metastasis group and the non-metastasis group according to whether there was lymph node metastasis.Clinical data and CT image features of the two groups were compared.The preoperative diagnostic value of CT image features for lymph node metastasis in colorectal adenocarcinoma was analyzed.Results:The short diameter of lymph node,proportion of oval lymph nodes,proportion of lymph nodes with uniform density,proportion of lymph nodes with smooth edges,and proportion of lymph nodes with homogeneous enhancement in the metastasis group were smaller(lower)than those in the non-metastasis group(P<0.05).Multivariate Logistic regression analysis results showed that short diameter,shape,and edge of the lymph node were factors influencing lymph node metastasis in colorectal adenocarcinoma(P<0.05).ROC analysis showed that the AUCs of short diameter,shape,edge,and enhancement of the lymph node for diagnosing lymph node metastasis in colorectal adenocarcinoma were 0.675(0.594~0.749),0.658(0.577~0.734),0.649(0.567~0.725)and 0.647(0.565~0.723),respectively.The AUC of joint diagnosis was 0.749(0.672~0.816),which was significantly greater than that of the short diameter,shape,edge,and enhancement of the lymph node(Z=2.082,2.907,3.003,3.052,P<0.05).Conclusion:CT for lymph node metastasis can assist in preoperative diagnosis of lymph node metastasis in colorectal adenocarcinoma.关键词
