Epidemiological Investigation on Animal Brucellosis Associated with a Clustering Human Outbreak

Abstract
On April 3,2024,a clustering outbreak of human brucellosis was found in a subdistrict in Shandong Province.In order to identify possible source of the pathogen leading to the outbreak,an epidemiological investigation was carried out on the sheep farms associated with the outbreak and all other households keeping sheep and cattle in the same subdistrict through on-site investigation,access to livestock breeding archives,screening sampling and laboratory testing.The results showed that the positive rate of sheep Brucella antibodies was 75.74%(178/235)on sheep farms associated with the human case,specifically,100%for genetic rams,89.04%for dairy sheep,76.56%for lambs,and 61.8%for replacement ewes/fattening sheep,indicating a statistical difference among these flocks(P<0.01);two positive cases were detected from another household that was epidemiologically associated with the farm,while none from other households in the same village and subdistrict where the farm was located.It was investigated that the animal case was more likely to be caused by brokers who purchased sheep carrying Brucella;most of the sheep sold by t he farm were intended for slaughter,there was thereby low risk of disease continuous transmission to other animals,but high risk of human cases due to consumption of raw milk,as the positive rate was high in dairy sheep.The shortcomings,such as inadequate supervision of animal health at grassroots level,lack of awareness campaigns,negligence in implementing preventive measures,delayed disease reporting by livestock farms/households,poor awareness of brucellosis prevention and control,etc.,were revealed by the outbreak.It was recommended to strengthen the supervision towards animal movement,to organize farms/households to participate in awareness improvement about targeted prevention and control measures,and to improve the capability and level of brucellosis prevention and control at grassroots level,for the purpose of safeguarding public health safety.关键词
