Staging and Phased Treatment of Idiopathic Membranous Nephropathy(IMN)Based on the Theory of"Yang Transforming Qi,Yin Forming Substance"

Abstract
Idiopathic membranous nephropathy(IMN)presents a pattern of gradual decline in Yang and increase in Yin when patients have not received steroid and immunosuppressant therapy.During the phase of urine turbidity,the body's Yang Qi is impaired by external wind-cold,leading to a gradual decline in Yang transformation of Qi.In the edema phase,Yang Qi weakens further,and water and dampness accumulate,leading gradual increase of Yin formation.In the kidney failure phase,Yang Qi has been exhausted,and Yang transformation is insufficient.Stagnation of blood stasis,dampness,and toxins leads to obstruction of the kidney collateral,worsening the dysfunction of Yang Qi transformation.This creates a complex pattern of both deficiency and excess,which becomes the root cause of the disease's chronic and difficult-to-cure nature.However,during treatment with steroids and immunosuppressants,the condition presents a pattern of excessive Yang and insufficient Yin.Based on the theory of"Yang transforming Qi,Yin forming substance,"the paper proposes that clinical treatment of IMN should be staged and phased.For patients not treated with steroids and immunosuppressants,the treatment should focus on warming and tonifying Kidney Yang.In the urine turbidity phase,the treatment should aim to warm the kidneys,tonify Qi,and stabilize the essence.In the edema phase,the treatment should focus on warming the kidneys,promoting diuresis,and draining dampness.In the kidney failure phase,the treatment should aim to warm the kidneys,open the channels,and promote the generation of new Qi.For patients undergoing steroid and immunosuppressant treatment,the treatment should focus on nourishing Yin,promoting diuresis,and clearing fire.关键词
