混合痔患者术后尿潴留的影响因素分析
Analysis of Influencing Factors for Postoperative Urinary Retention in Patients with Mixed Hemorrhoids

Abstract
Objective:To investigate the risk factors for postoperative urinary retention in patients with mixed hemorrhoids,and to construct and internally validate a predictive model.Methods:A total of 517 patients with grade Ⅲ-Ⅳ mixed hemorrhoids who treated at Yichang Central People's Hospital from January 1 to May 31,2023 were selected.Patients were divided into a urinary retention group(n=86)and a non-urinary retention group(n=431)based on whether postoperative catheterization was required.Lasso regression and multivariate Logistic regression were used to analyze the independent influencing factors for postoperative urinary retention in patients with mixed hemorrhoids,and a nomogram prediction model was constructed and internally validated.Results:Lasso regression screened out 11 characteristic variables.Multivariate Logistic regression analysis identified male(OR=2.588,95％CI:1.526,4.389),body mass index(BMI)≥24 kg/m2(OR=5.254,95％CI:3.028,9.116),intravenous fluid volume on the day of surgery＞700 mL(OR=2.207,95％CI:1.194,4.078),spinal anesthesia(OR=4.058,95％CI:2.295,7.175),and postoperative pain＞5 points(OR=2.114,95％CI:1.115,4.010)were independent risk factors for postoperative urinary retention in patients with mixed hemorrhoids(all P＜0.05).A nomogram prediction model was constructed and the receiver operating characteristic(ROC)curve was plotted,with the area under the joint prediction curve(AUC)being 0.799.Calibration curves before and after Bootstrap internal validation indicated that the risk fitting curve was close to the ideal curve.When the probability of urinary retention occurrence was between 0％and 95％,the model had good clinical applicability.Conclusion:Male,BMI ≥24 kg/m2,intravenous fluid volume on the day of surgery＞700 mL,spinal anesthesia,and postoperative pain＞5 points are independent risk factors for urinary retention.The constructed nomogram prediction model has good discrimination,calibration,and clinical applicability.关键词
