Differentiation and treatment of refractory diabetic foot ulcers based on"consumptive disease causing dry blood and hidden pathogens causing collateral blockage"
摘要
Abstract
Refractory diabetic foot ulcers(DFU)represent one of the most prevalent chronic complications of diabetes mellitus.DFU patients face high risks of limb amputation and mortality.Traditional Chinese medicine categorizes DFU under by foot ulceration with progressive tissue necrosis.Pathological manifestations include muscle putrefaction,blood desiccation,tendon-bone destruction,and eventual visceral decay leading to death.The pathogenesis of refractory DFU involves chemical damage and a deficiency of qi and bodily fluids.Yin fire descends,disrupting the scattered energies.This condition leads to dry blood,necessitating detoxification,and constriction of the tendons and collaterals.Treatment should prioritize the following:clearing and nourishing the source of qi,promoting the circulation of qi and fluids,eliminating excess heat,cooling the body and blood,removing blood stasis,generating new energy,and facilitating the circulation of pathogenic factors while clearing the meridians.关键词
