从玄府失司、肺气不宣论治难治性慢性咳嗽
Treating refractory chronic cough from Xuanfu dysfunction and lung qi failure in dispersion

Abstract
"Xuanfu"was first seen in Huangdi Neijing and subsequently refined and developed by successive generations of medical scholars into the Xuanfu theory.This theory has been widely applied in diagnosing and treating clinical diseases.The Xuanfu theory emphasizes the importance of maintaining the patency of internal and external channels in the human body and the coordinated functioning of the zang-fu viscera.Our team posits that it is closely associated with the onset,development,and progression of refractory chronic cough.Starting from the Xuanfu theory and the physiological characteristics of lung qi,Xuanfu dysfunction and lung qi failure in dispersion are the key pathogenesis of refractory chronic cough.Specifically,the clear and deficient nature of the lung and the obstruction of Xuanfu lay the foundation for disease onset;external pathogenic factors invading internally,and the stagnation of phlegm,dampness,and blood stasis are the pathogenic factors;the depletion of lung qi and Xuanfu malnutrition constitute the ultimate outcome of the disease.Guided by the Xuanfu theory and the principles of the dispersion and descent of lung qi,we have established three core therapeutic strategies:expelling pathogens and tonifying qi to open Xuanfu,activating blood circulation and regulating qi to unblock Xuanfu,and nourishing yin and replenishing qi to restore Xuanfu.These methods can be applied at different stages of the development of refractory chronic cough,providing a novel approach for its diagnosis and treatment,and clinical efficacy can be improved.关键词
