Research progress of vascular smooth muscle cells in the pathogenesis of thoracic aortic aneurysm and dissection

Abstract
Thoracic aortic aneurysm and dissections represent a group of clinically prevalent cardiovascular disorders characterized by progressive dilation of the aortic wall.Its pathogenesis remains incompletely understood.Anatomically,the aortic wall consists of three layers,with vascular smooth muscle cells being the principal constituent of the middle layer.This review synthesizes recent domestic and international literature to examine the mechanistic contributions of vascular smooth muscle cells to aortic wall pathology and dissection development,while evaluating targeted preventive and therapeutic strategies directed at these cells.The findings demonstrate that thoracic aortic aneurysm and dissection pathogenesis induces phenotypic switching of vascular smooth muscle cells into functionally diverse subtypes.These transformed cells exhibit dual functionality:they respond to neural stimuli by secreting growth factors and cytokines including MMP,TGF-β and TNF-α that modulate disease progression,while simultaneously releasing collagen to facilitate aortic remodeling and repair.Therapeutic interventions for thoracic aortic aneurysms and dissections must prioritize inhibition of key genetic regulators governing vascular smooth muscle cell phenotypic transition,particularly upstream and downstream mediators such as MEKK2/3,ALDH2 and ANXA1.关键词
