机械瓣膜置换术后华法林稳定治疗剂量预测模型研究
Development of a prediction model for the stable therapeutic dose of warfarin after mechanical valve replacement

Abstract
Objective:This study aimed to develop an optimized model for warfarin dosing in patients post-artificial me-chanical valve replacement by analyzing multi-gene polymorphisms alongside clinical variables to enhance model accuracy and predictive capability.Methods:Clinical data from 522 patients were meticulously tracked and recorded,with gene polymorphisms identified through Sanger sequencing techniques.The impact of gene polymorphisms and clinical variables on the daily stable dose of warfarin was assessed using multiple linear regression analysis.During the stepwise regression process,multicollinear variables were systematically screened out to construct a computational model predicting stable warfarin doses.Clinical efficacy performance was evaluated based on the desired predicted percentage.Results:A total of 297 patients were included for model inference and validation purposes.The newly developed model is expressed as follows:Y^=0.048-0.012(age)+1.512(BSA)-0.812(rs9923231 AA)+1.811(rs9923231 GG)+1.581(rs1057910 AA)-1.090(rs1057910 AG)-0.159(rs699664 AA).This model accounts for approximately 60%of the variance observed in individualized drug dosing requirements.Conclusion:GGCX(rs699664)may serve as a potential predictor for determining warfarin dosage;our newly established model holds promise for guiding personalized warfarin administration within clinical practice among the southern Chinese population.关键词
