Abstract
Tunneled cuffed catheter(TCC)is a type of vascular access for hemodialysis patients.During clinical use,it is prone to catheter dysfunction,which may lead to inadequate hemodialysis or even prevent it from being carried out.Therefore,understanding the prevention and treatment measures and risk factors for TCC dysfunction is of great significance.This article reviews the prevention and treatment measures and risk factors for TCC dysfunction based on recent domestic and foreign research literature.The literature review results show that excessively high or low hemodialysis blood flow rate,hypersensitive C-reactive protein(hs-CRP),low pre-dialysis central venous pressure(CVP),the position of the arterial external connector close to the outside of the TCC,long-term indwelling of temporary catheters,left internal jugular vein catheters,hypoproteinemia,low initial blood flow(BF),high hemoglobin(Hb),high hematocrit(HT),and short activated partial thromboplastin time(APTT)are risk factors for TCC dysfunction.Among them,elevated hs-CRP is an independent risk factor for TCC dysfunction.Various catheter sealing techniques(such as heparin,argatroban,urokinase,citrate,taurine,etc.)and oral anticoagulant/antiplatelet drugs(such as clopidogrel,warfarin)can effectively prevent thrombosis,etc.For already occurred central venous occlusion,it can be recanalized through minimally invasive methods such as the Surfacer® Inside-Out® system.This is of great significance for preventing the occurrence of TCC dysfunction in clinical practice and improving the quality of life for patients.关键词
