重复外周磁刺激联合任务导向性训练对脑卒中患者上肢运动功能及运动诱发电位的影响
Effect of Repetitive Peripheral Magnetic Stimulation Combined with Task-Oriented Training on Motor Function and Motor Evoked Potential of Upper Limbs in Stroke Patients

Abstract
Objective To observe the effects of repetitive peripheral magnetic stimulation(rPMS)combined with task-orient-ed training on upper limb motor function and motor evoked potential(MEP)in stroke patients.Methods A total of 40 stroke pa-tients who met the inclusion criteria were enrolled in the Department of Acupuncture and Rehabilitation of the First Affiliated Hospi-tal of Anhui University of Chinese Medicine from January 2023 to March 2024,and were divided into control group and experimen-tal group according to the random number table method,with 20 cases in each group.Both groups were given symptomatic drug treatment and conventional rehabilitation intervention.Besides,the control group was given task-oriented training and rPMS"sham-stimulation"treatment,and the experimental group was given rPMS combined with task-oriented training.The treatment was admin-istered once daily,5 times/week,for 4 consecutive weeks.The Modified Ashworth Scale(MAS)was used to evaluate the muscle tone of elbow flexor and wrist flexor muscles before and after treatment;the short-form Fugl-Meyer Assessment for Upper Extremity(FMA-UE)scale was used to evaluate the upper limb motor function of patients;the Modified Barthel Index(MBI)was used to as-sess the patients'activities of daily living;and the MEP amplitude and latency of the abductor pollicis brevis muscle on the affected side were measured to evaluate the excitability of the cerebral cortex on the affected hemisphere.Results(1)Comparison of mus-cle tone of the elbow flexor and wrist flexor muscles:Compared with pre-treatment,MAS scores of the upper limb elbow flexor and wrist flexor muscles decreased after treatment in both groups(P<0.05);compared with the control group,MAS scores of the elbow flexor and wrist flexor muscles in the experimental group decreased more significantly after treatment(P<0.05).(2)Comparison of upper limb motor function:Compared with pre-treatment,FMA-UE scores increased after treatment in the two groups(P<0.05);compared with the control group,FMA-UE score increased more significantly in the experimental group after treatment(P<0.05).(3)Comparison of activities of daily living:Compared with pre-treatment,MBI scores of the two groups increased after treatment(P<0.05);compared with the control group,MBI score of the experimental group increased more significantly after treatment(P<0.05).(4)Comparison of excitability in the cerebral cortex of patients:Compared with pre-treatment,MEP amplitude of abductor pollicis brevis muscle on the affected side increased(P<0.05)and the latency period shortened(P<0.05)in both groups after treat-ment;compared with the control group,the experimental group had a significantly larger MEP amplitude and shorter latency of ab-ductor pollicis brevis muscle on the affected side after treatment(P<0.05).Conclusion rPMS combined with task-oriented training can enhance cortical excitability of the affected hemisphere,reduce upper limb muscle tone,improve upper limb motor function,and improve activities of daily living in post-stroke patients.关键词
