失效模式与效应分析在静脉用药调配中心普通胰岛素调配全流程风险管理中的应用研究
Application of failure mode and effect analysis in risk management of the entire insulin deployment in PIVAS
摘要
Abstract
Objective:To strengthen the risk management of the entire insulin deployment in PIVAS.Methods:To conduct the hospital failure mode and effect analysis(HFMEA)in the complete process of insulin deployment in PIVAS.Select failure modes with medium and above risk levels,sort out and analyze their failure causes,propose rectification measures,and perform HFMEA again on each failure mode after rectification.Results:In the whole process of PIVAS insulin deployment,17 failure modes of high and medium risk levels were selected,and rectifications were made from several major aspects,such as processes,HIS system,and quality control management.After intervention,the total RPN value of the 17 failure modes decreased from 1 080 to 356,which was a decrease of 67.0%.Conclusion:Applying HFMEA to the risk management of PIVAS insulin throughout the entire process can effectively improve the safety of insulin deployment,and strengthen the risk management of PIVAS insulin deployment throughout the entire process.关键词
