Orthopedic surgical robot-assisted navigation for precise screw placement in the treatment of femoral neck fractures in young and middle-aged patients

Abstract
Objective:To investigate the efficacy of precision screw placement under the assistance of TiRobot® orthopedic surgical robot navigation system in the treatment of femoral neck fractures in young and middle-aged patients.Methods:60 patients with femoral neck fractures who were admitted to Honghui Hospital Affiliated to Xi'an Jiaotong University from January 2021 to March 2023 were selected.Based on different surgical approaches,patients were divided into the experimental group(n=30)and the control group(n=30).The experimental group underwent robot-assisted navigation for precise screw placement,while the control group received conventional freehand screw placement.Comparisons were made between the two groups regarding operative time,number of guidewire drillings,intraoperative fluoroscopy frequency,anteversion angle,neck-shaft angle deviation,intraoperative blood loss,postoperative femoral neck shortening,hip joint function during follow-up,and fracture healing time.Results:Compared to the control group,the experimental group exhibited significantly better outcomes in operative time,number of guidewire drillings,intraoperative fluoroscopy frequency,neck-shaft angle deviation,and anteversion angle deviation(P<0.05).Statistically significant differences were observed in postoperative Harris scores and intraoperative blood loss between the two groups(P<0.05).The experimental group demonstrated shorter fracture healing time and less postoperative femoral neck shortening than the control group(P<0.05).No severe complications such as incision infections or internal fixation failure occurred in either group,and all fractures healed during follow-up.Conclusion:Compared to conventional freehand screw placement,robot-assisted precision screw placement for femoral neck fractures in young and middle-aged patients demonstrates significant clinical efficacy with lower surgical complexity,shorter operative time,fewer guidewire drillings and intraoperative fluoroscopy exposures,it could minimize ionizing radiation exposure for surgical staff,accelerate fracture healing,and reduce complications,which can be recommended for widespread application.关键词
