机器人辅助椎弓根螺钉内固定术治疗强直性脊柱炎合并胸腰椎骨折的临床效果
Clinical efficacy of robot-assisted pedicle screw fixation in the treatment of thoracolumbar fractures in patients with ankylosing spondylitis

Abstract
Objective:To evaluate the therapeutic efficacy of robot-assisted pedicle screw fixation for thoracolumbar fractures in patients with ankylosing spondylitis(AS).Methods:A total of 84 AS patients with thoracolumbar fractures who were treated at the Sixth Affiliated Hospital of Xinjiang Medical University from January 2020 to January 2024 were randomly divided into the control group(n=42)and the observation group(n=42).The control group underwent manual pedicle screw fixation aided by surgical robotic arms or optical navigation systems,while the observation group received robot-assisted pedicle screw fixation.Perioperative indicators,radiological outcomes,functional recovery indicators,complication rates,bone healing,and implant failure rates were compared between the two groups.Results:Compared with the control group,the observation group had a longer operative time(P<0.05)but higher radiation exposure,Cobb angle correction rate,vertebral height restoration rate,and proportion of grade 0 screw placement accuracy(P<0.05).Additionally,less intraoperative blood loss,fewer fluoroscopy sessions(P<0.05),and a shorter bone healing time(P<0.05)were found in the observation group.At 3 months postoperatively,American Spinal Injury Association(ASIA)Impairment Scale showed a decrease in grade D and an increase in grade E,with the observation group having a higher proportion of grade E(P<0.05).The observation group was also showed lower overall complication rates,implant failure rates,and ODI scores at 3 and 12 months postoperatively(P<0.05).Conclusion:Robot-assisted pedicle screw fixation can significantly improve surgical outcomes in AS patients with thoracolumbar fractures by enhancing screw placement accuracy and biomechanical stability,thereby promoting neurological recovery and reducing complications.Despite increased radiation exposure and prolonged operative time,its clinical benefits(particularly in complex anatomical conditions)outweigh potential risks.This technique represents a precise and safe treatment option for AS patients with thoracolumbar fractures and warrants broader clinical adoption.关键词
