单侧与双侧穿刺经皮椎体成形术治疗胸腰椎转移瘤伴病理性骨折的疗效比较

Abstract
Objective To compare the efficacy of unilateral and bilateral percutaneous vertebroplasty(PVP)in the treatment of thoracolumbar metastatic tumors with pathological fracture.Methods The 58 patients who suffered from thoracolumbar metastatic tumor with pathological fractures were divided into unilateral group(The 31 cases were per-formed with unilateral PVP)and bilateral group(The 27 cases were performed with bilateral PVP),according to dif-ferent puncture methods.The surgical status,pain VAS,ODI,the percentage of anterior height of injured vertebrae and kyphotic Cobb angle of injured vertebrae were compared between the two groups.Results All patients were fol-lowed up for 3 months.The filling volume of bone cement,intraoperative fluoroscopy frequency,operation time in uni-lateral group were less(shorter)than those in bilateral group(P<0.05).Pain VAS and ODI in both groups:They were decreased at 3 d and 3 months postoperation,compared with the preoperation(P<0.05);there were no statisti-cal differences between the two groups(P>0.05).The percentage of the anterior edge height of injured vertebrae and the kyphotic Cobb angle of the injured vertebrae:At 3 d and 3 months postoperation,both groups were improved than the preoperation(P<0.05);there were no statistical differences between the two groups at 3 d after operation(P>0.05);and 3 months after operation,bilateral group was better than unilateral group(P<0.05).Conclusions Both unilateral and bilateral PVP puncture can achieve satisfactory analgesic effect in the treatment of thoracolum-bar metastatic tumor with pathological fracture,but bilateral puncture has the advantages of less height loss of injured vertebra and better recovery of spinal deformity.关键词
