Abstract
Objective To investigate the effect of percutaneous transforaminal endoscopic discectomy(PTED)in the treatment of lumbar disc herniation(LDH)complicated with nerve root canal stenosis.Methods According to differ-ent treatment methods,65 middle-aged and elderly patients with LDH complicated with nerve root canal stenosis were divided into PTED group(The 34 cases were treated with PTED)and unilateral biportal endoscopy(UBE)group(The 31 cases were performed with UBE technique).The operative time,intraoperative blood loss and length of hos-pital stay were recorded.Lumbosacral angle and lumbar curvature were measured to evaluate lumbar stability.The pain VAS was used to evaluate pain,the lumbar function was evaluated by ODI scores,and the operative effect was evaluated by modified MacNab evaluation standard.Results All patients were followed up for 6 months.Operation time,intraoperative blood loss and hospital stay in PTED group were less(shorter)than those in UBE group(P<0.05).Pain VAS and ODI scores:At 3 d,3 and 6 months after surgery,the data were decreased in both groups,com-pared with the preoperation(P<0.05),and the improvement of PTED group was more obvious than UBE group(P<0.05).Lumbosacral angle and lumbar curvature:At 3,6 months after surgery,the two groups were improved than the preoperation(P<0.05),and PTED group was better than UBE group(P<0.05).There was no statistical difference in the excellent-good rate of the effect between the two groups(P>0.05).Conclusions PTED technique for LDH combined with nerve root canal stenosis has the advantages of short operation time,less intraoperative blood loss,short hospital stay,less damage to spinal stability,and better lumbar function recovery.关键词
